What do the young adults

who live in the Leaving

Care Project say about it?

`Before coming to the Leaving Care Project I didn’t want to go to school but the staff in the project encouraged and supported me in getting an education and a steady routine and I’m glad they did.’

`Through the project I have learned to communicate better and my confidence has been built up.’

`If there is an emergency and I feel lonely at night I can phone the on-call service any time, day or night, 365 days a year.’

`I met others with similar experiences to myself and I’ve made new friends.’
`It has its ups and downs but I really enjoy the relationship I have with my project worker.’

 

So it sounds too good to be true?

Well, there are rules! You make agreements when you come in. It covers things such as:

the time you come into your flat at night

a the fact that you are not allowed
drugs on the project premises

if you are under 18 no alcohol is allowed in your flat

r young adults have to have a job or be in training – no staying in bed all day

  1. the project workers have a master key. They can come into your flat in an emergency, or if they feel something is wrong

i information is shared among staff, so when things go wrong at night or during the weekend the on-call staff will know you and you will know them

m the project has found that the rules ensure that everyone is safe. Some young adults have no problem keeping them and appreciate the support they get from Leaving Care. Some young adults have found that the rules

don’t suit them. If you don’t keep the rules or break your contract you may lose your flat.

If after considering the issues carefully you think the project might suit you, contact your social worker.

What will happen then?

  • Your social worker will need to refer you to the Project. You can come and see the Project and see if it would suit you. Then you can start to meet staff more regularly to prepare for a move.

support plan with the Project.

LI A meeting will be held where your report is agreed and you will be given a flat.

 

What do the young adults

about the assessment?

In your assessniUnt you work one-to-one with your project worker: you’re getting to know your project worker and they’re getting to know you.’

`My project worker listens to me, she always remembers what I have talked about, and this makes me feel there is someone who wants to listen to me.’

The risk assessment identified my problems and it makes sure these get worked on.’

`You talk about things you need support with e.g., cooking, budgeting and maintaining my flat. This can be challenging but I get support from my Project Worker.’

‘I was nervous, we discussed my worries and fears, the Project meets my needs, I am independent but I still have the support I need.’

`I felt good when I completed my assessment I feel as if I can talk to the staff, before coming to the project I didn’t want to talk to anyone.’

Project is a step closer to being independent. Self-contained/shared accommodation in the community is provided for young adults. Along with the accommodation a project worker provides practical and emotional support.

Have a life outside the project, but don’t be afraid to get involved and get along with the rest of the young people.

Young adults can live in their own accommodation or in their family home and receive the same intensive support from a project worker.

Aftercare

Aftercare Support is available to all young adults who have moved on from the project. It includes befriending and support in

crisis and emergency situations. These services are provided as requested by the young adult.

太好了,无法令人相信,是吗?

是有规则的! 参与计划是要签协议。 内容包括:

晚上回住所的时间

您不能在处所吸毒

不满18岁的话,您在处所不能喝酒

您必须有工作,或参加培训  – 不能整天呆在床上

项目工作人员有总钥匙。 遇到紧急情况或觉得情况不对时,他们可以进入您的处所

工作人员之间共享信息,晚上或周末出了什么事的话,呼叫台的工作人员认识您,您也认识他们。

项目的规则保证人人都很安全。 遵守规则对一些青年来说不是问题,他们很珍惜离家看护所提供的支持。  但也有些青年觉得这些规则不合适。

如果您不遵守规则或违背合约,您可能失去您的住所。

在考虑周全之后,如果您认为项目适合您,请与您的社工联系。   

下一步该做什么?

  • 您需要社工把您转介给该计划。 您可以来,看看该计划是否适合您。    之后您就可以定期与工作人员见面,准备搬进来。

您要同意合约的条款,以及项目的支持计划。

然后会与您见面,并分给您一处居所。
